Authorization for Use of Visual Materials 1
The undersigned grants non-exclusive international and electronic license to the Montana Natural Heritage Program (MTNHP)
to use, reproduce, distribute, display, and publish certain (see attached list) slides, photographs, color transparencies,
images, information or portions thereof ("Materials") in digital or print media which the Grantor provides to the MTNHP.
Grantor represents, to the best of Grantor’s information, that:
Grantor is the holder of the copyright to the Materials and has the right to authorize the use indicated above;
The provision of Materials to the MTNHP does not infringe any statutory copyright, common law right, or any proprietary right of any third party.
Grantor agrees that the MTNHP may publish the Materials on its electronic online site(s). A copyright notice will appear
on all MTNHP publications, and credit will accompany any reproductions made by MTNHP from the Materials.
Grantor acknowledges that third parties may copy Materials from the MTNHP electronic online site(s).
Notice: the copyright law of the United States (Title 17, U.S. Code) governs the making of reproductions of copyrighted materials.
It is the responsibility of the user of the Material to understand and abide by the copyright and the user is liable for any infringement.
Grantor agrees to release The Montana Natural Heritage Program, its officers, directors, agents, employees, and those acting on its
behalf from all debts, claims, and liability of any kind arising out of or in connection with the use of the Materials.
Grantor further agrees to indemnify the MTNHP against any liability arising from any claims of copyright ownership or
infringement or breach of another's rights by any third party.

1 Please note that the form must be completed by the copyright holder.
If an individual has created the Materials as a part of his or her employment, then the employer may be the
copyright owner unless the employee has an agreement to the contrary. In the case of employer ownership,
the individual who has the authority to authorize on the behalf of the employer must fill out the form.
In the case in which a governmental body is the employer, you will have to determine who is authorized by state
law to give authorization.
2 Copyright is assumed to reside with the photographer or illustrator unless
otherwise stated. If an additional credit or source is to be cited, please indicate.
3 Creative Credit: Provides artist recognition when copyright resides with an
institution, organization or business.
